The Community Treatment Team provides an assessment and treatment service for people who are experiencing difficulty with their mental health.
The team is made up of a number of health care professionals. The Community Treatment Team provides a service for men and women over the age of 18 years who require a period of assessment and treatment.
How to get referred
All referrals to our working age (18-65 years) adult community mental health teams in Northumberland will be sent to our single point, where referral will be triaged. A referral template has been developed for inclusion in patient information systems which you can access here.
All referrals using this template should be addressed to our secure, dedicated nhs.net single point of referral address firstname.lastname@example.org
Each referral will be processed via our multidisciplinary referral triage HUB, which is manned daily between 10am and 12 noon with rostered clinical and admin staff from across the county. Working in conjunction with Consultant Psychiatrists, Pharmacists, Nursing Staff, OT’s and administrative personnel, we will aim to provide the quickest possible response to all referral requests, identifying the most appropriate course of action, including assessment, advice, medication queries, signposting and onward referral.
